Prediabetes Diet: The Clinical Plan to Reverse It
Prediabetes means an HbA1c of 5.7%–6.4% or fasting glucose 100–125 mg/dL. About 96 million U.S. adults have it, and 70% will progress to type 2 diabetes without intervention (CDC, 2024).
The good news: prediabetes is the most reversible stage of metabolic disease. The Diabetes Prevention Program (DPP) showed that diet + activity reduces progression to T2D by 58% — beating metformin (31%) (Knowler et al., NEJM).

What Drives Prediabetes
- Insulin resistance in muscle and liver
- Visceral fat producing inflammatory cytokines
- Refined carbohydrate intake creating chronic insulin spikes
- Sedentary behavior reducing muscle glucose uptake
- Sleep deprivation (<6 hr) impairing insulin sensitivity by ~30%
Diet addresses 4 of the 5.
The Core Principles
- Lower glycemic load, not necessarily low carb
- 30 g protein at every meal
- 30–40 g fiber/day, mostly from vegetables and legumes
- Time-restricted eating — 12 hour overnight fast minimum
- 2 servings/week fatty fish
- Walk after every meal (10 min minimum)
Foods to Build Your Plate Around
Non-starchy vegetables: leafy greens, broccoli, cauliflower, peppers, zucchini, asparagus, mushrooms, tomatoes
Lean proteins: eggs, chicken, turkey, fish (salmon, sardines, cod), Greek yogurt, cottage cheese, tofu, tempeh
Legumes: lentils, chickpeas, black beans, edamame
Intact whole grains: steel-cut oats, quinoa, barley, farro (½ cup cooked portions)
Healthy fats: avocado, EVOO, nuts, seeds, fatty fish
Low-GI fruit: berries, apples, pears, citrus, kiwi
Foods to Eliminate or Strictly Limit
- Sugar-sweetened beverages (including 100% fruit juice)
- White bread, white rice, instant oats, breakfast cereal
- Pastries, cookies, candy, sweetened yogurt
- Processed meats (sausage, bacon, deli meats)
- Deep-fried foods
- Alcohol — more than 1 drink/day for women, 2 for men

What Reversal Looks Like
In the DPP study, participants averaged a 7% weight loss and 150 min/week of activity. HbA1c typically falls 0.3–0.5% in 3 months and another 0.3% by month 6.
Frequently Asked Questions
Can prediabetes be reversed in 3 months? A1C is a 90-day average — meaningful reductions show up at the 3-month mark, but most people need 6–12 months to fully normalize.
Is fruit OK with prediabetes? Whole fruit yes (berries, apples, pears, citrus); fruit juice and dried fruit no.
Is intermittent fasting good for prediabetes? A 12–14 hour overnight fast is well-tolerated and improves insulin sensitivity. Extended fasting (>18 hr) should be physician-supervised.
Should I cut carbs entirely? No. The Mediterranean and DPP-style moderate-carb diets outperform very-low-carb diets for long-term adherence and A1C reduction.
