Creatine monohydrate is the most studied sports supplement in existence — over 1,000 published trials — yet women were historically under-represented in that research. That gap has closed considerably since 2021, and the findings are consistent: women benefit from creatine at least as much as men, and possibly more in midlife.

Creatine is stored in muscle as phosphocreatine, which rapidly regenerates ATP during high-intensity effort. More phosphocreatine means more total work per session — an extra rep or two on each set. Across months, that additional training volume is what produces strength and lean-mass gains. Creatine is not a hormone or a stimulant.
The body produces roughly 1 g/day endogenously, and omnivores obtain another 1–2 g from meat and fish. Vegetarians and vegans have measurably lower muscle stores and typically show the largest response to supplementation.
Meta-analyses show creatine plus resistance training increases lean mass by roughly 1–2 kg over 4–12 weeks versus training alone, with upper-body strength gains of 5–15%. The mechanism is training capacity, not water weight alone — you cannot skip the training and expect the result.
Practical framing: creatine is a multiplier on a resistance-training programme, not a substitute for one.
This is the most interesting recent development. In a 2-year randomized trial in post-menopausal women, creatine combined with resistance training preserved femoral neck bone mineral density and increased femoral shaft width versus placebo (Chilibeck et al., Med Sci Sports Exerc 2015). Creatine alone, without training, did not do this.

The brain uses phosphocreatine too. Randomized data show creatine improves cognitive performance most under stress conditions — sleep deprivation, mental fatigue, and in vegetarians with low baseline stores (Avgerinos et al., Exp Gerontol 2018). Small trials also suggest an adjunctive antidepressant effect at higher doses (5–10 g/day), though this is preliminary and should not replace treatment.
Limited but promising: creatine has been shown to improve insulin sensitivity markers when combined with exercise, which is directly relevant to PCOS-related insulin resistance. Pair it with the nutrition approach in the PCOS meal plan. Evidence here is early-stage — treat it as a possible secondary benefit, not a reason to start.

Timeline: 1–2 kg of scale weight in the first two weeks is intracellular water — expected and desirable. Strength changes typically show at weeks 3–6.
Creatine pulls water into muscle cells, not under the skin. The visual result is fuller, firmer muscle, not softness. To gain visible muscle size, most women need a calorie surplus and years of progressive training; 3 g of a molecule your liver already makes will not do it accidentally. If the scale moves 1 kg in week one, that is cell hydration and it plateaus.

Will creatine make me gain weight? Expect 0.5–1.5 kg of intracellular water in the first fortnight. It is not fat, and it plateaus.
Do I need to cycle off? No. Long-term daily use is well tolerated in studies up to five years.
Should I take it on rest days? Yes — the goal is saturated muscle stores, not an acute effect.
Is creatine safe during perimenopause? Yes for most women, and it may be more useful then given declining muscle and bone mass. Confirm with your clinician.
Does creatine work without exercise? Muscle and bone benefits require resistance training. Cognitive effects may not.
