There is no single Hashimoto's diet. What consistently helps is separating levothyroxine from coffee, calcium, iron and fibre, correcting low ferritin, vitamin D and selenium status, eating enough protein, and testing suspected trigger foods one at a time instead of eliminating everything at once.

Levothyroxine and food timing

Take levothyroxine on an empty stomach with water. Wait 30–60 minutes before coffee, and separate calcium, iron, soy, high-fibre meals and antacids by about 4 hours.

TPO antibodies

Raised thyroid peroxidase antibodies confirm the autoimmune mechanism but are not the treatment target; clinicians dose on TSH and free T4 plus symptoms.
